Job Description
As a Case Management Support Nurse (PhRN), you will play a crucial role in coordinating patient care and ensuring the delivery of high-quality healthcare services. You will collaborate with the healthcare team to develop and implement effective care plans for patients, utilizing your clinical and administrative skills.
Your responsibilities will include assessing patient needs, developing individualized care plans, coordinating with healthcare providers, and monitoring the progress of patients’ treatment plans. You will also provide patient education and support, helping them navigate through their healthcare journey.
To qualify for this position, you should hold a valid PhRN license and have experience in case management or a related field. Excellent communication, organizational, and problem-solving skills are essential. You should be able to work well in a fast-paced environment and manage multiple tasks efficiently.
