Job Description
Responsibilities:
- Efficiently coordinate and schedule nursing visits and other clinical services for home health patients
- Collaborate with healthcare team to develop patient care plans
- Review medical records to ensure proper care delivery
- Act as a key contact for patients and caregivers
- Maintain accurate scheduling records
- Address scheduling conflicts and challenges promptly
- Ensure compliance with regulations and company policies
Qualifications:
- Must be a Registered Nurse
- 1 year of Home Health scheduling experience
- Proficient in medical records review
- Strong organizational skills
- Familiarity with Home Health regulations
- Attention to detail
- Open to night shifts and immediate start
