Job Description
We are looking for a Care Navigator Team Lead to manage a team that coordinates patient journeys, oversees referrals, and ensures timely access to services. You will mentor navigators, monitor performance metrics, streamline workflows, collaborate with clinical teams and community partners, and manage complex cases requiring escalation.
