Job Description
We are seeking a dedicated Case Coordinator to manage client cases and ensure services are delivered effectively. The ideal candidate will have strong organizational skills, attention to detail, and the ability to communicate efficiently with clients and stakeholders.
Responsibilities include coordinating client care plans, monitoring progress, and acting as a liaison between clients and service providers. Previous experience in case management or a related field is preferred.
