Job Description
A Case Manager is responsible for assisting and supporting individuals or families in need. They work closely with clients to identify their needs and develop appropriate plans to address them. The main role of a Case Manager is to facilitate and coordinate services, ensuring that clients are connected to the resources and support they require.
Key responsibilities:
- Assessing client needs and developing individualized service plans
- Coordinating with various service providers to ensure access to necessary resources
- Advocating for clients and representing their interests
- Monitoring and evaluating the progress of clients towards goals
- Providing ongoing support and guidance to clients throughout the process
Strong communication, organizational, and problem-solving skills are essential for success in this role. A Case Manager needs to be empathetic, compassionate, and able to build trusting relationships with clients.
