Job Description
We are currently seeking a highly motivated and skilled Case Triage Nurse (PhRN) to join our team. As a Case Triage Nurse, you will play a crucial role in providing expert and accurate telephonic assessment and triage of patients’ medical conditions and appropriate level of care.
Responsibilities:
- Conduct comprehensive patient assessments over the phone, gathering relevant medical information and determining the urgency of the situation
- Apply clinical knowledge and critical thinking skills to make appropriate recommendations for care, including referrals, diagnostic tests, and treatment plans
- Consult with healthcare professionals, including physicians, to ensure appropriate care coordination
- Document all triage calls accurately and efficiently
Requirements:
- Valid PhRN license and relevant nursing degree
- Ability to assess and prioritize patients’ needs in a fast-paced environment
- Excellent communication and interpersonal skills to effectively communicate with patients and healthcare professionals
- Strong critical thinking and problem-solving abilities
