Job Description
Are you passionate about assisting individuals with chronic illnesses in effectively managing their conditions and improving their quality of life?
We are currently seeking a skilled and dedicated Chronic Case Manager to join our team. As a Chronic Case Manager, you will be responsible for coordinating holistic care plans for patients with chronic illnesses, ensuring they receive the necessary medical attention, support, and resources to effectively manage their conditions on an ongoing basis.
This role involves collaborating with a multidisciplinary team of healthcare professionals, including physicians, nurses, social workers, and other support staff, to develop individualized care plans that address the specific needs and goals of each patient.
Key responsibilities:
- Assessing patient needs and developing comprehensive care plans
- Coordinating appointments and referrals to specialists
- Providing education and support to patients and their families
- Monitoring patient progress and adjusting care plans as needed
- Ensuring timely and accurate documentation of patient interactions and interventions
