Job Description
As a Clinical Care Manager (CCM), you will play a vital role in ensuring the delivery of high-quality care to our patients. You will be responsible for coordinating and managing the overall clinical care of our patients, working closely with healthcare professionals and interdisciplinary teams.
Key responsibilities include:
- Developing and implementing care plans based on patient needs and goals
- Monitoring patient progress and providing ongoing support and guidance
- Collaborating with healthcare providers to ensure effective care coordination
- Evaluating patient outcomes and making necessary adjustments to treatment plans
- Providing education and resources to patients and their families
Requirements:
- Minimum Bachelor’s degree in Nursing or a related field
- Valid license to practice as a Registered Nurse (RN)
- Strong clinical knowledge and experience
- Excellent communication and interpersonal skills
- Ability to multitask and prioritize effectively
