Job Description
A Medical Case Manager is responsible for coordinating and managing the medical care of patients within a healthcare facility. They work closely with doctors, nurses, and other healthcare professionals to ensure that patients receive the appropriate medical treatment and resources.
Key responsibilities include:
- Assessing the healthcare needs of patients and developing individualized care plans
- Coordinating appointments and medical procedures
- Collaborating with healthcare providers to ensure the continuity and quality of care
- Identifying and addressing any barriers to medical care
- Educating patients on their health conditions and treatment options
- Monitoring the progress and outcomes of patients’ medical care
To be successful in this role, candidates should have strong communication and organizational skills, as well as a solid understanding of medical terminology and procedures. A degree in nursing or a related field is typically required.
