We seek an experienced Intake Specialist to facilitate the intake process for new clients, requiring excellent organizational and communication skills.
Responsibilities:
- Manage intake process and document collection.
- Assess client eligibility for services.
- Ensure smooth transitions and ongoing care coordination.
- Effective communication with clients and internal teams.
- Maintain accurate client records.
- Collaborate for process enhancement.
Requirements:
- Home health experience.
- Track record in intake coordination.
