Job Description
Community Engagement:
- Identify and engage communities in the program’s catchment area.
- Utilize different community engagement approaches and participatory rural appraisal (PRA) tools to develop and maintain strong relationships with communities.
- Identify stakeholders and facilitators in the communities, conduct regular meetings, and maintain positive working relationships with them.
- Have a good understanding of the telehealth program and its operations.
- Collaborate effectively within a team.
- Establish community referrals to the telehealth facility and programs.
- May perform other tasks of RN, NA, pharmacy assistant, and receptionist if required.
Health Awareness:
- Spread information about the telehealth center and make referrals accordingly.
- Conduct health awareness sessions in communities, homes, and clinic settings.
- Identify community health needs and provide information to the core team.
- Travel extensively to community homes, locations, NGOs, and outreach destinations.
- Collect community feedback regarding services provided.
- Attend training sessions and weekly meetings.
Data Reporting:
- Document all data, including qualitative and quantitative information, from the field.
- Report progress and weekly updates to the program coordinator.
Miscellaneous Duties:
- Perform miscellaneous job-related duties as assigned.
- Support other departmental public health programs in establishing community linkages.
- Conduct disease screening.
- Willingness to travel to different program locations when needed.
