Job Description
- Participate in clinical rounds with Consultants and coordinate with floor Doctors and Staff.
- Supervise and assist in patient Pharmacy Services inventory management; collect information required for interventions.
- Review, monitor, and control drug profile and Computerized Physician’s Order Entry (CPOE) of all admitted patients.
- Contact physicians in case of inaccuracy of order/non-availability of medicine or alternative.
- Assist in Antibiotic stewardship round and monitor high alert medication dispensation.
- Document interventions daily and present relevant issues to Manager, Pharmacy Services.
- Compile and prepare monthly presentations on Effective Interventions.
- Develop strong communication between Pharmacies and floor staff.
- Identify and correct HMIS related problems and provide summary reports to Manager Pharmacy.
- Review and monitor stock of all crash trolley medicines and provide monthly reports.
- Provide current and accurate drug information to health care professionals.
- Record drug information data and prepare monthly graphical reports.
- Provide technical and administrative support to Pharmacy staff and training to interns.
- Provide counseling on safe drug use to Doctors, staff, patients, and families.
- Arrange Continuing Medical Education programs for Pharmacy staff and other healthcare professionals.
- Monitor and record all non-formulary drugs arranged for inpatients.
- Perform assigned tasks from Manager Pharmacy or Assistant Manager Pharmacy.
