Job Description
Job Summary: As a Care Coordinator, you will oversee and coordinate patient care to ensure high-quality services. You will work with patients, providers, and stakeholders to create care plans, monitor progress, and advocate for patients.
Responsibilities:
- Coordinate with healthcare providers and community resources.
- Create care plans to address physical, emotional, and social needs.
- Advocate for patients’ rights and preferences.
- Educate patients on conditions and self-care strategies.
- Monitor progress and maintain documentation.
- Participate in quality improvement initiatives.
- Adhere to regulatory requirements and ethical standards.
Qualifications:
- Bachelor’s degree in relevant field.
- 2-3 years’ healthcare experience.
- Strong clinical knowledge and communication skills.
- Proficiency in EHR and healthcare regulations.
