Job Description
- Ensure safe, appropriate, and cost-effective use of medicine to enhance patient safety.
- Plan, organize, and oversee the delivery of clinical pharmacy services at DHQ Hospital Badin (Managed By: IHHN).
- Provide services to medical & surgical specialties, participate in consultant rounds.
- Monitor patients, collect specifications, conduct interviews, and identify proper therapy.
- Improve compliance, educate patients, attendants, doctors, nurses, and paramedical staff.
- Perform medicine reconciliation on admission, evaluate appropriateness, and identify interactions.
- Enter detailed notes in medical profiles, design pharmacotherapy plans, and justify recommendations.
- Provide drug information, train pharmacy staff, maintain formulary, and review requests.
- Monitor crash trollies, conduct educational sessions, and prepare monthly reports.
