Job Description
Requirements: 2-3 Years experience as a Clinical Pharmacist. Experience in Gynae & Obs/Neonatology is preferred. Fixed morning shift.
Duties:
- Provide Clinical Pharmacy Service to various Medical and Surgical Specialties, participate in consultant ward rounds.
- Work collaboratively with healthcare professionals to enhance patient safety and ensure safe Medicines Management practices.
- Review patient profiles daily, identify and resolve prescription errors, document clinical interventions.
- Monitor patients, collect data, perform assessments, interview patients, and set therapy goals.
- Educate patients/nursing staff on medication regimes, achieve medicines reconciliation on admission.
- Evaluate pharmacotherapy, design comprehensive plans, justify recommendations based on evidence-based information.
- Enter detailed notes in patient records, provide drug information to healthcare professionals.
- Participate in training pharmacists, pharmacy technicians, and residents.
