Job Description
- Participate in clinical rounds with Consultants, coordinate with floor Doctors and Staff.
- Supervise patient Pharmacy Services inventory management; collect information for interventions.
- Review, monitor and control drug profile and CPOE for all admitted patients.
- Participate in Antibiotic stewardship round, contributing to patient care.
- Monitor high alert medication dispensation and provide proper administration information.
- Document interventions, present issues to Manager, Pharmacy Services daily.
- Prepare presentations on Effective Interventions monthly.
- Assist in hospital formulary development and implementation at TIH.
- Develop communication between Pharmacies and floor staff.
- Identify and correct HMIS problems, provide reports to Manager Pharmacy.
- Review crash trolley medicines stock, present monthly reports.
- Provide current drug information to healthcare professionals.
- Record drug information data, prepare monthly graphical reports.
- Provide support and training to Pharmacy team members.
- Offer counseling for safe drug use to Doctors, staff, patients and families.
- Arrange CME programs for healthcare staff on rational drug use.
- Monitor and record non-formulary drugs for inpatients.
- Perform tasks assigned by Manager Pharmacy/ Assistant Manager Pharmacy.
